REQUIREMENTS

Original/Certified True Copy of Oath of Office
Certification from the ff. Officials as to the inclusive dates of Services Rendered with Satisfactory Performance and shall include the date and place of birth of Barangay Officials (Applicant) using the revised CSC Form dated April 2012:

A. Punong Barangay

B. DILG-Municipal

C. DILG-Provincial

D. DILG-Regional

Appointment as Barangay Treasurer/Secretary Resolution duly concurred by Barangay Treasurer/Secretary
Authenticated copy of Birth Certificate and Marriage Contract (if applicable)

QUALIFICATIONS

FOR BARANGAY OFFICIALS

A. Completed the term of office

B. In cases where services rendered by elected/appointed officials fall short as to the completion of term of office, the deficiency on the number of days shall not be more than fifteen (15) days

C. In cases of appointive barangay officials who were not able to complete the number of years of service corresponding to the term of office of their appointing Punong Barangay for more than fifteen (15) days, service rendered by the said appointive barangay officials under previous appointments of similar capacity in the same Barangay only, may be considered to be able to complete the required number of years corresponding to the term of office of the concerned appointing Punong Barangay

FOR SANGGUNIANG MEMBERS

A. SANGGUNIANG MEMBER FIRST LEVEL ELIGIBILITY

Have served as Sanggunian Member for an aggregate period of six (6) years; and
Have completed at least seventy-two (72) units leading to a baccalaureate/bachelor's degree

B. SANGGUNIANG MEMBER SECOND LEVEL ELIGIBILITY

Have served as Sanggunian Member for an aggregate period of nine (9) years; and
Have completed as baccalaureate/bachelor's degree
